This was quite the surprise, I went searching for visual novels to play and I found this one called Katawa Shojo. And its story is centered around this guy who has a heart arrhythmia. These are the screenshots I’ve taken so far that mention his heart (which also follows a soft red flashing and a heart sound effect after the dialog).
