Since women’s bodies in extended families are likely pretty similar, they were likely to undergo similar gestational changes. (e.g. my mother, aunt and daughter couldn’t poop either, lol.) If one woman was physically different than the others or had notably-different pregnancy-experiences, they’d go find her “someone else” to talk to, hence the doula and midwife came into being. These women were the experts until “science” (men) took childbirth away from women.
