3-holes-2-tits: Not only a bit of predicament, being kept on tip-toes by the tits (tit-toes anyone?), but also well tied arms, a tight hood that looks like it blocks out both all light and some sound, and to tip it off, possibly a vibrator pushed up against the warm lips. The predicament itself might at first look like it is a struggle between pain in the breasts and pain in the feet from the posture (no way to really find any kind of position to rest in), but the real evilness for a multiorgasmic being is the addition of an unforgiving vibrator. Either as a constant tease never strong enough to bring out that needed release, or as a too powerful tool that it is more or less impossible to get away from and that sooner or later turns into a painful annoyance.
