In patients who already have very bad kidney function, vancomycin can push them over the edge, into acute kidney injury (AKI) or end-stage renal failure (ESRD). For particularly sick patients, there may not be a good alternative to vancomycin, so the medicine team and I have to weigh the pros and cons carefully. -- source link
